Today, OAMES received the final drafts of the revised certificates of medical necessity forms (CMNs) for DME items from the Ohio Department of Medicaid. These forms are effective for use on July 16, 2018. The 21 fillable-PDF files were distributed to OAMES members last evening and will soon be posted on the association's website. Please discard any previous drafts of these forms you may have received in the past OAMES or ODM.
ODM staff has indicated that the CMNs are scheduled for posting to the Department’s website on Friday, July 13. Those forms are located here: http://medicaid.ohio.gov/Provider/ProviderTypes/HomeandDurableMedicalEquipmentProviders
These CMNs accompany the Department's numerous revised DME rules in the ODM section of Ohio Administrative Code 5160-10. OAMES will continue to work with ODM staff during the transition of the rules and CMNs so watch for updates going forward.
